Entry & Nomination Forms

Entries and nominations for the 2019 Central Otago Awards are now open.

On this page you'll find the entry and nomination forms for each category. Each form includes eligibility criteria, terms and conditions, and how to submit your final entry. If you have any questions, please contact the Awards Team at centralotagoawards@codc.govt.nz or phone 03 440 0642.

Entries and nominations must be received by 5pm, Thursday 2 May 2019.

Award categories include:

Business Excellence Award
Celebrating businesses with outstanding business practices.

Young Professional Award
Celebrating young people, aged between 21 and 35, who are achieving outstanding results in their chosen career. 

Apprentice Award
Celebrating apprentices who consistently produce excellent results both in practice and theory assignments.

Community Service Award
Celebrating individuals who have made a significant contribution to the well-being of the Central Otago district and its people.

Youth Award
Celebrating exceptional young people in our community, aged 13 to 20 years.

Nomination - Business Excellence Award


Typically businesses don't like to 'blow their own trumpet' and enter themselves for an award. So this year we're giving you the opportunity to nominate a business that is achieving great things in our region, to encourage them to enter the Business Excellence Award category.

Anyone can nominate - employees, associates, friends, family, clients and customers. Once we receive your nomination we will contact the business, send them an entry form and invite them to enter.

Nominations are confidential but you are welcome to let the business know you have made the nomination and encourage them to enter.

Nominations can be emailed to centralotagoawards@codc.govt.nz right up until entries and nominations close on 2 May 2019. But remember, don't leave your nomination too late! The business you nominate will need to accept the nomination and complete the entry form to enter the awards so you need to leave them enough time to do so.

Entry - Business Excellence Award


The Business Excellence Award is about celebrating business excellence and achievement, and recognising outstanding business practices. The Award is open to businesses of all sizes and from any industry.

Entry into the Business Excellence Award consists of completing a short entry form and attending an interview with the Business Judging Panel. In the interview you'll have the opportunity to meet with the judges and talk about your business and what makes it outstanding. Included on the entry form is a list of the sorts of things the judging panel may ask, to help with your preparation.

Eligibility
The business must be:
- Independently owned.
- Based in Central Otago district.

Young Professional Award

The Young Professional Award is about recognising talented young people who are making a significant contribution in their place of work or business they are building. It also acknowledges their contribution to the local community.

It is about celebrating a young person who is achieving outstanding results in their chosen career.

What are the Judges looking for?
The Judges will be looking for evidence of the positive impact this person is having on your business in terms of leadership and work ethic. As well as appropriate examples of how the nominee is embracing our regional values in their work place or community.

Eligibility
The individual must be:
- Nominated.
- Aged 21 – 35 years old as of 15 May 2019.
- Living and working in Central Otago district.

Apprentice Award


The Apprentice Award for Overall Excellence is about promoting outstanding training and personal achievement of apprentices working in any industry and to raise awareness of career opportunities.

It is about recognising apprentices who consistently produce excellent results both in practice and theory assignments.

What are the Judges looking for?
The Judges will be looking at things such as, quality of work, long term industry potential, initiative, overall training performance and X-factor. As well as appropriate examples of how the nominee is embracing our regional values in their work place and training.

With your entry, you will need to include a short statement from the apprentice detailing how the apprenticeship has helped his / her career.

Eligibility
The apprentice must be:
- Completing an apprenticeship and be completing a National Certificate in their chosen field.
- Working in the industry at the time of nomination.
- Living and working in Central Otago district.

Community Service Award


The Community Service Award is about recognising individuals who have made a significant contribution to the well-being of the Central Otago District and its people over a significant period of time or through exceptional actions.

It’s about celebrating those with big hearts who dedicate time to helping others.

What are the Judges looking for?
The Judges will be looking for insights into the depth or scope of the nominee’s contribution to their community, as well as appropriate examples of how the individual integrates our regional values into their lives.

Eligibility
The individual must:
- Normally live in Central Otago district. However, if a person from outside the district boundaries carried out an exceptional action within the district, then they can be deemed eligible for receipt of an award at the discretion of the Judges.
- Not have received a Royal or National honours for the service.
- Not be a current serving political figure.

Youth Award


The Youth Award is about celebrating exceptional young people in our community.

It recognizes those who are taking a lead in their community using their skills to make a difference beyond what is expected of them – these young people have the ability to motivate, engage and positively influence others.

What are the Judges looking for?
The Judges will be looking for good examples of the young person’s great leadership skills, as well as appropriate examples of how the individual embraces our regional values in everything they do.

Eligibility
The individual must be:
- Aged 13 - 20 years old as of 15 May 2019.
- Living in Central Otago district.
